What Does a Support Coordinator Do?

At Unity SCS, our Support Coordinators work alongside you, your family, and your community to help you get the most out of your NDIS plan.

We support you to:

Understand your NDIS funding
Set up your services and service agreements
Connect with the right providers and community supports
Build everyday skills like budgeting and planning
Navigate challenges and avoid or manage difficult situations

Bringing Your Plan to Life

We support you to understand your NDIS plan and put it into action with the right services and supports.

This includes:

Helping you find and choose providers that suit your needs
Assisting with setting up service agreements
Helping you organise your initial bookings

We support you to understand and stay on top of your NDIS funding so you can use it with confidence.

This includes:

Building your confidence to manage and track your NDIS budget

Connecting You to Community Supports

We help you find and access services in your community that are not funded by the NDIS but still important for your wellbeing.

Our Support Coordinators follow the NDIS Code of Conduct and respond promptly to any safety concerns.

Who Can Access Support Coordination

Support is available for people with disability, mental health needs, or those who require extra help because of age, illness, or injury.

What Does Support Coordination Cost?

Standard Support Coordination: $100.14 per hour
Specialist Support Coordination: $190.54 per hour

Additional costs, such as travel time, may apply.

If you have NDIS funding, these costs may be covered depending on your plan.
